The CHIP is a community-driven roadmap for improving health across the North Shore.

How the Community Health Cycle Works:

The Community Health Cycle is a two-step process that helps us understand changing community health needs, with community input at the center.

  • Step 1: Community Health Assessment (CHA)

    We gather data and feedback through surveys, interviews, and research to identify key health needs and priorities.

  • Step 2: Community Health Improvement Plan (CHIP)

    Using CHA findings, we create a plan to address priority health issues and drive meaningful change.

Traditionally done in 4 to 5-year cycles, this work is now becoming a living process- one where NSHD can be flexible, update our focus, and work regularly so we can stay connected and responsive to the community’s needs.
