Reportable Disease Information for Health Care Providers

Reporting Requirements:

Under Wisconsin law, certain communicable diseases are reportable and must be reported to the North Shore Health Department when the individual resides in the North Shore.

In addition, health care providers and required reporters must follow state reporting timelines and procedures:

Who Must Report:

The following providers and organizations are required to report communicable diseases, including but not limited to:

Nurses

Laboratories

Physicians and physician assistants

Dentists and dental providers

Pharmacists

Therapists (physical, occupational, speech)

Social workers and counselors

Hospitals, clinics, and hospices

Emergency medical services and ambulance providers

Long-term care and residential facilities

Schools and child care settings (as applicable)

Reporting Timeframes:

Category I Diseases

Immediate Reporting -Category I diseases are of urgent public health importance.

Report immediately by phone

In addition to the phone report, submit a case report within 24 hours using:

  • Wisconsin Electronic Disease Surveillance System (WEDSS)

    or

  • Fax case report to (414)371-2988

Category II Diseases

Must be reported within 72 hours of recognizing a confirmed or suspected case.

Reports may be submitted through:

Sexually Transmitted Diseases:

Sexually transmitted diseases must be reported within 72 hours of identifying a confirmed or suspected case.

Reports may be submitted:

Public Health Follow-Up:

Once a report is received, public health staff may:

  • Review case information

  • Contact providers or facilities for follow-up

  • Provide guidance, education, or other public health interventions as needed
